Release of Liability & Participant Agreement

By signing below, I understand that training at Deadweight Martial Arts involves physical activity, close contact, and instructor-led drills that may push my limits. I acknowledge that injuries can happen—including bumps, bruises, strains, or more serious occurrences—and I accept that risk as part of the training process.

I agree that I’m responsible for listening to my body, communicating any injuries or concerns, and training in a way that supports my safety and the safety of others. I understand that I can sit out, modify, or ask for help at any time

I hereby release Deadweight Martial Arts, its owners, instructors, staff, agents, and affiliates from any and all liability for any injury, loss, or health issue that may occur during my time on the mats, in the gym, or at any event connected to Deadweight Martial Arts.

Medical Authorization

If I am injured and unable to respond, I give permission for staff or emergency personnel to provide care or call for assistance. I understand that I’m responsible for any medical costs that may arise.

Photo & Video Consent

I understand that classes or events at Deadweight Martial Arts may be photographed or filmed for training or promotional purposes. I consent to the use of my likeness unless I opt out in writing.

Respect & Conduct

I agree to follow gym rules and respect the space, staff, and fellow students. I understand that if I act in a way that puts others at risk or disrupts the environment, I may be asked to leave.

For Parents or Guardians of Minors (under 18)

I am the parent or legal guardian of the participant listed below. I have read and agree to the terms of this waiver and give my full permission for their participation.

Deadweight Martial Arts – Payment Agreement

This agreement outlines the payment terms and conditions for your membership with Deadweight Martial Arts, located at 258 Marcus Garvey Blvd, Brooklyn, NY 11221. By signing below or completing online registration, you agree to the following:

1. Membership Type and Rate

You are enrolling as a Early Member at the special rate of $150 per month.

Billing begins on the 1st of each month, unless an alternate start date is requested in writing.
If you join after the 1st, your first payment will be prorated to cover the remaining days in that month. Your regular monthly billing will then begin on the 1st of the following month.

This promotional rate is available only to early signups and is billed automatically each month unless cancelled.

2. Billing and Renewal

Memberships automatically renew every 30 days on the 1st of the month.
Payments are processed through the Gymdesk billing portal using the card or payment method you provide.

If your payment fails, you’ll receive a notice via email. Please update your billing method within 5 days to avoid suspension of access.

3. Cancellations

There is no long-term contract — you may cancel your membership at any time by logging into your member portal or contacting the gym directly.

If you cancel before the end of a billing period, your membership will remain active until that period expires.

A $30 early cancellation fee applies to memberships canceled before the first 3-month minimum term.

4. Refunds

As one of our Early members, your support helps us grow. While payments are typically non-refundable, we always aim to be fair and understanding. If something comes up, just let us know — we’ll do our best to take care of you.

5. Updating Your Membership

Members may update or change their plan (e.g., upgrade, family add-on, or pause) through the online member portal or by contacting the gym.

Any rate changes will be communicated at least 30 days in advance.

6. Agreement and Acknowledgment

By signing below, you acknowledge that:

• You have read and understood this payment agreement.

• You authorize recurring monthly payments at the rate listed above.

• You agree to the cancellation and refund terms described.
